Detection of acute cellular rejection after liver transplantation is difficult. Although the use of autologous antibody as a scintigraphic agent has been discussed, it is difficult to induce hyperacute rejection in animal models. We attempted to detect acute cellular rejection after liver transplantation by radioimmunoscintigraphy using radiolabeled rabbit IgG monoclonal antibody (MAb) against human HLA-DR1. Five-milliliter blood samples were taken 1 h after injection of 5 to 6 mCi of 131I-MAb. The recipients were selected to undergo early acute rejection; 3 of them underwent an orthotopic liver transplantation, and the other 2 had a second liver transplantation; 1 of them received immediate-type donor liver transplantation, and the other had a delayed graft function. Nine of the 13 liver transplant recipients were successfully given a liver scan by intravenous injection of 131I-MAb. The scintigraphic images revealed significant increases in radiolabeling uptake in the spleen, liver, heart, bone marrow, and kidneys in 3 recipients, and in the transplanted liver in 3 recipients who underwent the second liver transplantation. However, only one patient with liver rejection showed an increased uptake in the liver with the scintigraphic image. There was no evidence of a greater effect of 131I-MAb in the donor liver with respect to the in vivo adsorption rate than in the nontransplanted liver in the 5 recipients with immediate-type donor liver transplantation.
